Appointments
To make an appointment, please call 817-424-FOOT (3668). If you can't keep an appointment, please give us one day's notice. Despite appointment schedules, emergencies can cause delays. If you appointment is affected, we will attempt to notify you.

Insurance
We participate with most insurance plans. Some plans require a referral which can be obtained from your family physician. In most cases, you will be responsible for a co-pay, co-insurance or deductible, and some charges not covered by your insurance. We will do our best to let you know ahead of time.

Payment Policies
Payment is due at each visit. We accept cash, checks, and credit cards. If you cannot pay at the time of service, please talk to our business manager and arrange special payment options.
